Application Process:
Call the center and provide diaper sizes/formula type for each child. A time will be scheduled for curbside pick up.
Documents Required: Diaper sizes and formula info, phone number, city of residence.
Eligibility Requirements: Parents/guardians with babies up to age 3 that live in Pottawatomie County and surrounding cities.
Payment/Insurance Accepted: Please contact provider for accepted forms of payment.
ADA Access: Access w/o Special Facilities, Designated Parking, No Stairs in Service Delivery Areas, Outside Ramp
